Strategies of increasing contraceptive prevalence implemented by nursing staff: Cross-sectional study in clinical base in the Karisimbi health zone, Provincial Division of health North Kivu, DRC.

Abstract:-

In the Democratic Republic of Congo, family planning is part of integrated health services. However, often policymakers and service providers are not taking family planning as their priority but focus their attention and resources on other health problems. The result is a 5.8 percent modern contraceptive prevalence rate in the country (although higher in Kinshasa where it is 14.1%).In addition, according to the magazine of the DSRP of the Republic. Democratic Congo on 8 and 9 March 2010 the health, the total fertility rate remains high at 6.3 in 2007 against 7.1 in 2001 and will remain always high for a number of years according to the estimated calculations of the projection of the fertility rate. The low use of contraceptive methods, particularly modern methods, can be mentioned as one of the factors that explain the high fertility in the country. The use of maternal health services seems to have significantly improved in recent years. However, there are still serious problems in the quality of services and the promotion of good governance in family planning program.

Retrospective analysis of Anterior circulation aneurysm: A institute study

Abstract:-

BACKGROUND: Subarachnoid hemorrhage(SAH) from ruptured aneurysm carries poor prognosis; half of patients die within 1 month of hemorrhage. Approximately 15% of patients with aneurysmal SAH die before reaching the hospital. The initial and definitive management of ruptured cerebral aneurysm is evolving, with clipping and coiling being alternative treatments.

AIMS & OBJECTIVE: Our aim is to study outcome of surgery on ruptured aneurysms of anterior circulation and to study the factors influencing the same.

MATERIALS AND METHODS:A retrospective and prospective analysis of consecutive 110 patients comprising the Anterior communicating artery ( Acom ),Middle cerebral artery (MCA) ,the internal carotid artery (ICA),Anterior ccerebral artery(ACA),Posterior communicating artery(PCOM) who have undergone surgery at our institute from January 2015 till january 2019 was performed.We have included patients who presented with SAH, only WFNS grade 1 and 2 diagnosed with ACOM OR MCA OR ICA OR ACA OR PCOM aneurysm and then underwent surgical management and Excluded Giant aneurysm; size > 2.5 cm,Multiple aneurysms,Should not be in a patient suffering from any connective tissue disorder. Or any other systemic disease e.g. Polycystic kidney disease which are associated with a high number of concomitant intracranial aneurysms and Operated by coiling.Neurological outcome was assessed using modified Rankin’s scale (mRS) and post operative CTA.

Results:In our study,most common age group affected is 54.20 yrs.Aneurysms are common in female sex.According to criteria of size, 101 patients had aneurysm size of less than 14 mm.In anterior circulation aneurysm 20%  ICA,25.45%  MCA, 2.75% ACA, 40.90% ACOM, 1.81% DACA,9.09% PCOM presents.Vaospasm present in 14.2 % of patients detected radiologically.Intraoperative rupture was happened in 15.45% patients.Post operative complications were faced like infarction(8%), hydrocephalus(14.8%),icu stay <7 days in 82.7%,post op de-compressive craniotomy (1%),0n discharge-motor deficit(10%), speech deficit(6.1%), neurocognitive (6.7%), mortality(6.4%).

RETROSPECTIVE ANALYSIS OF BENIGN SPINAL CORD TUMORS :AN INSTITUITIONAL EXPERIENCE

Abstract:-

BACKGROUND:Spinal cord tumors account for about 15% of central nervous system neoplasm. Benign and malignant neoplasms can arise from intraspinal structures such as meninges, spinal cord, nerve roots, blood vessels and other tissues.1 These are 10 times less frequent than intracranial tumors with majority of them being benign.Space occupying lesions in the spinal canal cause compression of the structures with resultant neurological deficits.

AIMS AND OBJECTIVES:To study various clinical presentation radiological correlation,pathological nature, surgical approaches, morbidity, mortality, the recovery and improvement in follow up of patients operated for “Benign spinal tumors”

MATERIALS AND METHOD:In this prospective and observational study of 50 patient who Underwent surgery in the B.J.M.C,Government Medical College and Hospital, Ahemedabad, from August 2015 to December 2018 for “Benign spinal cord tumors”.Ethical clearance was taken from instituitional review committee.

OBSERVATION AND RESULTS:Our study shows incidence of  “ Benign spinal tumor”  was  maximum in age group 41-50 year with 20 %  and minimum in 31-40 year with 6%, male is 54% and in female is 46 % which was almost equal, Patients presented with weakness (48 %) as the most common complain followed by back pain (46%).Most common bowel-bladder symptom was urinary incontinence which was present in (8%) of patients, followed by retention(6%),constipation in(4%)  and fecal incontinence in 4%.Out of the 50 patients in this study 42 % had weakness in lower limbs, 6 % had weakness in upper limbs, 8 % had weakness in both and 44 % patients had no weakness. 46 % of our patients presents with hypertonia,32 % of patients presents with impaired touch, pain and temperature sensation and 12 % with impaired joint and position sensation. most common location of tumor is in dorsal region (30%).most common location of tumor is in IntraduralExtramedullary(IDEM) and then Intradural Intramedullary (IDIM). Benign spinal tumor causes displacement of the cord in 12 % of patient, compression in 52 % of patients, enlargement of the cord in 12 % and no effect in 24 % of patients.completetumor removal was achieved in 86 % of patients, while in 14% neartotal removal was done.Out of the total 50 patients, in 39 patients dura was closed primarily,while in 11 patients fascia lata graft was used for duralrepairOurstudyshows most common tumors are nerve sheath tumors,accounts for 32% of the total tumors. Next most common tumor is meningioma,which accounts for 20% of the tumors and epidermoid is 20%.72 % of patients improved neurologically while 8% patients deteriorated neurologically and 20 % of patients remain same as preoperative status of neurology.

CONCLUSION:This study of describing the demographic characteristics, histopathological features, anatomical location, and vertebral level of these tumors treated surgically.. Data in the current study represent the characteristics of primary spinal cord tumors in Asian countries.Tumour type and biology plays an important role, with benign IDEM tumours displaying the best prognosis for long term survival after complete excision. The pretreatment neurological status of the patient is important in determining outcome after treatment.Delayed presentation is the main reason for poor preoperative neurologic status, leading to poor outcome in the management of spinal tumors. A high level of suspicion and acknowledging the classical symptoms of cord compression are the most important factors in shortening the time to diagnosis of spinal tumors.

Study to Assess the Knowledge and Practices of Frontline Workers ( ANM’S) regarding Mission Indradhanush at Selected Health Centers, Hyderabad, Telangana

Abstract:-

Mission indradhanush has been launched to reach out to the children where routine vaccination cannot be reached out. Objectives 1. To assess & correlate the knowledge and practices regarding Mission Indradhanush among front line workers.  Non experimental Descriptive research design was used for this study and the data was collected by using self administered structured questionnaire and cheek list. Sample 138 were selected, the sample comprised of 138 ANM’s at selected 14 clusters of urban health centres. Results were described as ANM’S 75.5% were having average knowledge scores, Their practices were ANM’S 55.1% are having good expressed practices, ANMS knowledge is not significantly correlated with practice (sig. Value is 0.713 > 0.05). There is a need to increase awareness and knowledge about the benefits and importance of vaccination, as well as the harmful consequences of non-complete immunization. By providing health education regarding immunization since the time of antenatal period, It can remove the obstacles and unnecessary fears about vaccinations covers under mission indradhanush. Researcher can recommend the govt to plan and organise crash training for frontline workers to strengthen efficient utilization of mission indradhanush.

The buccal mucosa's epithelial cells are encircled by mucus, an intercellular ground substance that can range in thickness from 40 m to 300 m. The majority of mucus is produced by the minor salivary glands and sublingual glands, which are also essential for preserving the mucin layer over the oral mucosa. The goal of the current study was to create a novel mucoadhesive drug delivery system for nateglinide. The goal of this work was to transport the medicine unidirectionally to the target locations in order to create a non-invasive dosage form that is more efficient and has improved bioavailability. With the aid of mucoadhesive polymers and a variety of mechanisms, bioadhesive or mucoadhesive systems stick to the mucosal surface for an extended period of time. Polyacrylic acid (carbopol polycarbophil), chitosan, (Polymethyl vinyl ether/maleic anhydride copolymers), cholestyramine, HPMC, sephadex, sodium alginate, PEG, dextran, sucralfate, poly (alkyl cyanoacrylate), and polylactic acid are some of the mucoadhesive polymers utilised to create such systems. The electronic theory, which proposed an attractive electrostatic force between the bioadhesive substance and glycoprotein mucin network, is one of many ideas used to explain the mechanism of mucoadhesion. On the other hand, adsorption theory claims that mucoadhesion is aided by secondary forces such as hydrogen bonding and van der Waals forces. Wetting theory is based on the development of close contact between bioadhesive polymers and mucus layers, whereas diffusion theory proposes physical entanglement of mucin strands with flexible polymer chains. The GIT's rapid mucus production makes it challenging to keep polymers' effective mucoadhesion in place. The stomach's tendency to be hydrated also lessens a polymer's ability to adhere to living things.
